Programs educate veterans on their VA benefits as well as assist with reintegration.

The Department of Veterans Affairs (VA) Post-9/11 Military2VA (M2VA) case management program assists post-9/11 servicemembers as they transition from active duty. The program embeds a M2VA case management team of professionals at each VA medical center. The teams are trained to meet the unique needs of post-9/11 veterans.

In fiscal year 2021, more than 212,000 veterans were screened for case management and more than 38,000 post-9/11 veterans received ongoing case management services to help them navigate the VA health-care system and coordinate their care. Case management teams educate veterans and their families on VA eligibility, benefits and services, transition and reintegration assistance, identifying services and community resources.

In addition to the M2VA case management program, VA has a separate Liaison Program. VA Liaisons for Healthcare assist ill and injured servicemembers in their transition from Department of Defense (DoD) to VA health-care facilities. These liaisons are nurses and social workers that work both on site and virtually to coordinate VA care from a military treatment facilities and DoD installations. The program has helped more than 140,000 servicemembers transition from active duty since the program’s 2003 inception.

The liaisons work hand-in-hand with the M2VA case managers to provide servicemembers with comprehensive care and assistance during their transition. Collaboration between the Liaisons for Healthcare and M2VA programs improves access to care, services and benefits by engaging veterans early in the transition process.
